HOW TO REGISTER UNIQUE DISABILITY ID – UNDER NAGALAND GOVERNMENT

Unique Disability ID under Nagaland Government is a Unique ID for person with Disability. This project is being implemented with a view of creating a National Database for PWDs, and to issue a Unique Disability Identity Card to each person with disabilities. Not only will the project encourage transparency, efficiency, and ease in delivering government benefits to persons with disabilities, but it will also ensure uniformity.


The tracking of physical and financial progress of beneficiary at all levels of hierarchy of implementation from village level, block level, District level, State level and National level.

How to Register for UNIQUE DISABILITY ID UNDER NAGALAND GOVERNMENT

Four Types of Details are Required

  1. Personal Details including Address.
  2. Disability Details.
  3. Employment Details.
  4. Identity Details

a. Person with Disability will click on register link to register with UDID web portal.

b. Using credentials PWD logs in to system and click” Apply online for Disability Certificate” Reads instructions and fills up online application.

c. Upload color passport photo and other requisite documents like income proof, identity proof and SC/ST/OBC proof as required.

d. Submits data to CMO office/Medical Authority.

e. CMO office/medical authority verifies data.

f. CMO office/ medical authority assigns the concerned specialists for assessment.

g. Specialist Doctor assesses disability of PWD and gives opinion on disability.

h. Medical Board reviews the case and assign disability percentage. CMO office prepare Disability Certificate and generates UDID and Disability certificate.

i. UDID datasheet goes for UDID Card printing and card dispatched to PWD.

Card Benefits of UNIQUE DISABILITY ID UNDER NAGALAND GOVERNMENT

The UDID card shall bring a host of benefits to the Persons with Disabilities as given below:

Persons with disabilities will not need to make multiple copies of documents, maintain, and carry multiple documents as the card will capture all the necessary details which can be decoded with the help of a reader.

The UDID card will be the single document of identification, verification of the disabled for availing various benefits in future.

Documents Required for Online Registration

Scanned copy of recent color photo, image of signature (Optional), Address Proof (Aadhar/Driving License/State Domicile etc.), Identity Proof (Aadhar Card/PAN Card/Driving License etc.), Disability Certificate.

Who Can Apply

As per the person with disabilities act, 1995 only given disabilities type can apply for UDID card:

  1. Blindness.
  2. Low vision.
  3. Cerebral palsy.
  4. Locomotor Disability.
  5. Mental illness.
  6. Hearing Implement
